Rhinoplasty – Nasal Aesthetic Surgery

Nasal aesthetic surgery, namely “rhinoplasty”, has become one of the most preferred aesthetic procedures by women and men in recent years. The fact that the nose affects facial expression very much due to its central position on the face increases the person’s desire to have a natural nose that will best suit them.

Rhinoplasty enables to remove deformities that occur in the nose congenitally or as a result of subsequent trauma, but also to correct intranasal anatomical defects that cause nasal obstruction. A successful rhinoplasty allows patients to be happy when they look in the mirror, while also eliminating complaints due to nasal obstruction.

“Rhinoplasty which is one of the procedures I also perform the most and that I can clearly observe the satisfaction of my patients can provide eye-catching changes in the appearance of people. But, as I have said to all my patients, rhinoplasty need to be planned correctly. It is necessary to first complete the facial analysis of the patients, evaluate their skin structures and learn their expectations in order to make an artless and natural lose in compliance with other organs on the person’s face.”

What is Rhinoplasty?

The operations in which the shape of the nose is changed to best suit the person’s face and enable them to have a natural appearance, while also improving the respiratory functions of the nose, are called “nasal aesthetic surgery” or “rhinoplasty”.

Rhinoplasty can be performed with open or closed technique in line with the scope of the changes to be made to the noses of the patients and their skin structures. I will soon give some information about which methods should be preferred for the patients. However, I would like to emphasize once again that there may be situations where both techniques should be preferred. Therefore, I recommend individuals to make evaluations between surgeons who have experience in both open and closed technique when they select the surgeon for nasal aesthetic surgery.

Who Can Have Rhinoplasty?

  • People having problems of self-confidence and feeling unhappy due to the shape of their noses,
  • Having a structural defect that affects facial expression in the shape of nose,
  • Having realistic expectations about the surgery,
  • Having anatomical problems preventing breathing through the nose,
  • Having completed bone development,
  • Having suitable general state of health for the surgery,
  • Being non-smoker or quitting smoking within the recommended period,
  • Feeling ready for the surgery and being aware that they need to fulfill the requirements of the post-operative process can be considered as an eligible candidate for rhinoplasty.

What are the Changes that Can Be Made in the Nose with Rhinoplasty?

Thanks to rhinoplasty, which is performed by evaluating the general health, nose structure and skin features of the person according to their needs;

  • The nasal hump can be corrected,
  • The nose can be narrowed or widened,
  • Asymmetry problems between nostrils can be resolved,
  • Anatomical problems that make it difficult to breathe through the nose can be eliminated,
  • Deviated septum can be removed,
  • The nasal tip can be lifted or straightened.

The Importance of Facial Analysis Before Rhinoplasty and Natural-Looking Nasal Aesthetics

If you remember the past years, people who underwent a rhinoplasty had almost the nose with almost the same features. Small, upturned and crooked noses also necessarily gave the message, “I had a rhinoplasty” from far away. In recent years, this trend has become obsolete; the nose shapes that are more natural and in compliance with the other facial features of the person are now preferred.

Each patient must be evaluated separately in order to perform a more natural rhinoplasty. Factors such as the person’s age, gender, skin structure, expectations, and nose structure, as well as nose shape defects, may affect the procedures that should be performed during rhinoplasty. After all, our main purpose is not to highlight the nose, but to create changes that will make the face light up and highlight the gaze with rhinoplasty. Therefore, we consider many variables during the facial analysis, from the eye structures of our patients to their cheekbones, the distance between the nose and lip to the chin structure, the nasofrontal angle between the nose and forehead to the ratio between the ear and nose.

How is Rhinoplasty Performed? Is Open Technique or Closed Technique More Advantageous?

Rhinoplasty can be performed with both open technique and closed technique, mostly under general anesthesia. If radical changes will be made in the nasal septum, surgeons prefer open rhinoplasty technique as they will work more comfortably. However, we complete the rhinoplasty with closed technique unless our patients need to have major changes made in their noses. We determine the most ideal rhinoplasty technique for each patient in line with the data we obtained from our patient – doctor interviews, our examinations and facial analysis results before the nasal aesthetic surgery.

What and How is Open Rhinoplasty Performed?

Nasal aesthetic operations that are performed by removing the nasal skin after cutting the part called “columella” located in the middle of the nostrils are called open rhinoplasty. The open technique, which facilitates access to the nasal septum, makes patients feel uneasy especially about scarring. The truth of the matter is that there may be a scar in open rhinoplasty, but this scar is not at a level to disturb patients. This scar, which is colored in red in the post-operative period, will fade over time. Open rhinoplasty is the preferred nasal aesthetic technique when there are anatomical deformities in the nasal septum and serious corrections in the shape of the nose are required.

What and How is Closed Technique Performed?

The only difference between closed rhinoplasty and open rhinoplasty is the way it reaches the nasal septum. The columellar incision is made in open rhinoplasty, while no incision is made in the nasal skin in closed rhinoplasty. All procedures are carried out through the nostrils. Therefore, closed rhinoplasty is also known as “scarless rhinoplasty”. Closed rhinoplasty may be preferred in patients who do not have serious anatomical problems in their nose and do not require radical changes in the nose shape.

“I do not think it is appropriate to create a pros and cons list between open or closed rhinoplasty. Although the recovery time and post-operative complaints of those who underwent a closed rhinoplasty last shorter, it will not be reasonable to prefer closed rhinoplasty at this point if the patients’ needs will be met more smoothly with open rhinoplasty. It should be remember that these two techniques, which differ in the way they reach the nasal septum, have their own pros and cons. Experienced surgeons who have comprehensive knowledge of both techniques can perform surgeries that meet the expectations of the patients and increase their comfort. Eventually, patients should trust their physicians and leave their choice of technique to surgeons and focus on the outcome.”

Piezo Rhinoplasty

I can say that Piezo surgery has become the most popular rhinoplasty technique in recent years. Piezo provides precise cutting of the nasal bones via special tiny saws that are able to produce ultrasonic vibrations and select tissue and so, rhinoplasty can be performed with both open and closed technique. Piezo surgery, where only bones and hard cartilage are shaped without touching the vessels, tissues, nerves or muscles in the nasal septum, creates very good results in patients. During this technique that can be applied to all patients, the nose should be skeletonized well; that is, the bone structures should be opened so that they can be clearly seen. While doing so, tissues should be intervened in a respectful way by seeing the layer structures under the skin. Although the duration of surgery extends in new users of this technique, the time difference decreases as it is being used and when the user is an experienced person. Since Piezo surgery is a protective and visual technique, clear incisions can be made in every plan and post-operative complaints become less common.

Healing Process After Rhinoplasty

Patients are usually discharged on the same day after their general state of health is checked. Edema and bruising may occur in the first week after the operation. The degree of edema and bruising may vary depending on the scope of the procedures performed. These problems, which are likely to occur in the post-operative period, are valid for the nasal aesthetic operation performed with both open and closed technique, and the complaints disappear to a large extent after the first week.

Those who have a rhinoplasty should wait between 6 months and 12 months to take the final shape of their nose. Within that period, changes to the nose will fully fit and patients will be able to observe the most precise results.

Points to Consider After Rhinoplasty

The success of rhinoplasty depends on many different factors. The success of the outcome may be affected by the patient’s skin structure, age, general health status, and obedience to the post-operative directives as well as the pre-operation planning and the surgeon’s experience. Thus, I explain the points to consider after rhinoplasty in detail to all my patients to whom I applied a rhinoplasty.

To sum up briefly, I can give the following information about the points to consider after rhinoplasty:

  • The patient should lie with her/his head high the first week after the operation.
  • Eyeglasses should not be used for the first 3 months after the operation.
  • The patient should not be exposed to sunlight without applying suncream or wearing wide hats within 6 months after the operation.
  • High-intensity exercises that will increase the risk of bleeding should be avoided for a minimum of 3 weeks after the operation.
  • The nose should be protected from impacts after the operation.
  • Intranasal creams and sea – ocean water should be used if recommended by surgeons after rhinoplasty.
  • Smoking should be avoided during the healing process.

Frequently Asked Questions About Rhinoplasty

Is Rhinoplasty Painful?

One of the most curious questions for my patients who applied to me for rhinoplasty is whether rhinoplasty is painful or not. Since the patient is anesthetized during the operation, s/he does not feel pain but there may be minimal pain after rhinoplasty. The resulting pain can be relieved with analgesics and the comfort of the patients may be increased.

Is There Any Age Limit for Rhinoplasty?

Yes, of course there is. Since we intervene in the nasal septum during rhinoplasty, we look at whether the bone development of our patients has been completed. Even if we have to make very minimal interventions to some of our patients when their respiratory functions are damaged due to trauma, we prioritize the completion of bone development for rhinoplasty. In general, I can state that rhinoplasty can be applied safely after the age of 16 in girls and 17 in boys.

When to Go Back to Work After Rhinoplasty?

My patients can usually start work 7 days after rhinoplasty. However, the situation may change for those who are engaged in a job that needs physical effort. In such a situation, it may take 3 – 4 weeks to start working after rhinoplasty.

How Much Does Rhinoplasty Cost?

It would not be right to make a clear statement about the cost of rhinoplasty. The price policy of the hospital where the operation will be performed, the materials used during the operation, the patient’s nose structure, whether she/he have had a rhinoplasty in the past, whether it will interfere with other nasal pathologies during the operation, and the technique preferred for the success of the operation, may change the pricing of rhinoplasty.

Can Otoplasty be Performed Simultaneously with Rhinoplasty?

Otoplasty, eyelid aesthetics, bichectomy and concha bullosa surgery can be performed simultaneously with rhinoplasty. Combined aesthetic operations can be performed at once to improve the patient’s respiratory functions and increase the compliance between the facial features. What is important at this point is the needs, expectations, general health and preferences of the patient.

Does Rhinoplasty Leave a Scar?

The answer to this question may vary depending on which technique the rhinoplasty is performed with. There is no visible scar in closed rhinoplasty and may be an indistinct scar in open rhinoplasty, but it is necessary to consider the fact that wound healing can vary from person to person.

Can Rhinoplasty be Performed in Summer?

There is no season for rhinoplasty; it can be performed every season. However, patients who have had an operation in the summer months should be protected from sun exposure, should not wear sunglasses until complete recovery and should not enter the pool in the early period.
